You have access prior to your appointment to complete your new patient registration. You may access this portal at www.bts.ema.md. Your username will be the email address you provided to the office. If you do not have a password, you can set it by choosing forgot password.

Please arrive 15 minutes before your scheduled appointment.

You will need to bring the following information to your first visit:

• Photo Identification
• Insurance Cards
• Medical History
• Medication List
• Surgery History
• Podiatric History

Your estimated copayment, deductible, and/or coinsurance will be collected at every appointment. Our office accepts Visa, Mastercard, American Express, Discover, Checks, and Cash. There is a 3% service charge for all credit/debit card payments. Although we try to inform patients of network participation, please check with your insurance company if you are unsure of our network status.
